Thank you for choosing Podiatry Care of Maine for your podiatry care.
Please visit our PATIENT FORMS page to fill out both the New Patient Form and the Authorization Form. You may submit this directly to us via secure email or you can request a copy be sent to you via email. Please visit our contact page for address, phone number and email address.
The authorization form is part of the privacy practice act. It also authorizes the doctor to treat you, send information to your insurance company and to discuss your medical history with your physician and/or family members. If you have any questions, please feel free to call our office.
We require you to bring your insurance card (s) with you so that we may make a copy for our records. Many insurance companies require a referral by your primary care physician before you may see a specialist. Please check with your primary care physician to be sure that this has been done. The referral must be received by our office prior to your visit. If you need assistance with your referral, please notify our office. Please also bring a picture ID with you.
You may find directions to our office on the DIRECTIONS page. Our office is handicap accessible. The office is located on the 3rd floor with elevator available. When facing the building, with your back toward Forest Avenue, enter in the right side entrance. Free Parking is available in our parking lot in front of the building.
Our normal business hours are Monday through Thursday 9:00 am to 5:00 pm and Friday 9:00 am to 2:00pm.
24 Hour emergency coverage is available.
