Please visit our PATIENT FORMS page to fill out both the New Patient Form and the Authorization Form. \u00a0You may submit this directly to us via secure email or you can request a copy be sent to you via email. \u00a0Please visit our contact page for address, phone number and email address.<\/p>\n
The authorization form is part of the privacy practice act.\u00a0 It also authorizes the doctor to treat you, send information to your insurance company and to discuss your medical history with your physician and\/or family members.\u00a0 If you have any questions, please feel free to call our office.<\/p>\n
We require you to bring your insurance card (s) with you so that we may make a copy for our records.\u00a0 Many insurance companies require a referral by your primary care physician before you may see a specialist.\u00a0 Please check with your primary care physician to be sure that this has been done.\u00a0 The referral must be received by our office prior to your visit.\u00a0 If you need assistance with your referral, please notify our office. \u00a0Please also bring a picture ID with you.<\/p>\n
